Appointments
Please call (972) 712-1911 to schedule an appointment. We require
24 hour notice if you are unable to keep your appointment.
On your first visit to our office, you will be asked to complete
a patient information form which will be a part of your permanent
record to be kept on file at our office. We have provided you the
registration forms in Adobe PDF format.
You may print them out and bring them completed to speed up your
registration process.

Payment is expected at the time of service, including but not limited
to co-payment, co-insurance, deductible and non covered services.
We participate with most major insurance plans and will file your
insurance claims for you. Always bring your insurance card with
you at the time of your visit. If you have any questions regarding
our services or fees please ask us so that we may keep you informed.
